Two Simple Steps to Get Your Results

Register your DNA sample BEFORE you ship it.

Please register the barcode number located on DNA collection tube using our secure registration form. Your DNA sample is designed to be shipped and received anonymously. This barcode number is the only way to identify your sample at our laboratory as part of our security and de-identification process.

Please use this link or scan the QR code on your phone to register once you have received your collection kit.

Complete the Health Survey
CHS research is in its infancy. An essential part of our ongoing research is collecting data about the varying symptoms CHS patients experience.


To collect this critical information, we have created this health survey, which takes less than 5 minutes to complete.
